Chicago is the third-largest therapy market in the United States, and Illinois as a whole offers a compelling mix of urban demand and rural shortage areas. Whether you’re launching a telehealth-first practice in the Loop or opening an office in the suburbs, the state’s robust insurance infrastructure — anchored by BCBS of Illinois — makes insurance-based private practice highly viable.

This guide, written by Jason Roy and reviewed by licensed clinicians, walks you through every step. Illinois has a unique two-tier licensing system (LPC vs. LCPC), requires a PLLC for practice entities, and demands 3,360 supervised hours plus two national exams for independent licensure.

The Complete Illinois Private Practice Roadmap

Understand IL’s Two-Tier Licensing System

Illinois uses a two-tier system managed by IDFPR (Illinois Department of Financial and Professional Regulation):

LPC (Licensed Professional Counselor)

Entry-level license. You cannot practice independently or open a private practice. Must work under a qualified supervisor (LCPC, LCSW, psychologist, or psychiatrist).

LCPC (Licensed Clinical Professional Counselor)

The independent clinical license. Only LCPCs can open a solo private practice, diagnose, and treat mental health conditions independently.

Key Point: You need an LCPC to operate a private practice in Illinois. If you only hold an LPC, you must first complete your supervised hours.

Complete 3,360 Supervised Hours (LPC to LCPC)

As an LPC, you must accumulate:

  • 3,360 total hours of supervised clinical professional counseling.
  • At least 1,680 hours (50%) must be direct client contact (synchronous telehealth counts).
  • 1 hour of supervision per week minimum.
  • Hours must be completed in no less than 2 years.

Supervisor Qualifications: Must be an Illinois-licensed LCPC, LCSW, clinical psychologist, LMFT, or psychiatrist.

Pass Both Required Exams

Illinois is one of the few states requiring two national exams for LCPC:

  • NCE (National Counselor Examination) — required for LPC.
  • NCMHCE (National Clinical Mental Health Counseling Examination) — required for LCPC upgrade.

CE Requirements: LCPCs must complete 30 continuing education hours every two years, including 3 hours of ethics.

Education Change (July 2026): Starting July 1, 2026, new applicants must hold a graduate degree with at least 60 semester hours (up from 48). Current LPCs are grandfathered in.

Form Your PLLC (or P.C.)

Illinois requires therapists to form a Professional Limited Liability Company (PLLC) or Professional Corporation (P.C.). Standard LLCs are not permitted for IDFPR-regulated professions.

  • File Articles of Organization with the IL Secretary of State.
  • Include a purpose clause specifying the professional services.
  • Register the entity with IDFPR separately.
  • All members must hold valid professional licenses.

No Publication Requirement: Unlike New York, IL does not require newspaper publication for PLLCs.
